First, create a new page in your LibGuide. For multi-column pages, resize to one column. Create a Standard box then choose the Media/Widget option. Copy and paste this code into the box:

Open Qualtrics then:
- Click on the Distribute Survey tab
- Then click on the Survey Link button
- Copy the link.
Back in LibGuides, paste the link in after the "src=" (it will replace the xxx in red you see above). The final product will look something like this:

Click Save & Close then refresh your screen, and there's your survey!
Directions for LibWizard: create new slide; choose Embedded Media option; paste in code; click Save.
